When an incident affects evidence integrity, key material, provider transport, or a public claim, it is recorded here with a postmortem and the corrective action that passed its own evidence gate. A clean dashboard is never offered as proof of absence of incidents.
Incident historyNONE RECORDED
No production incidents have been recorded. Because the public surface is synthetic and the control core is not yet independently accepted, no live production traffic has run; absence of recorded incidents therefore reflects an inactive live surface, not a proven reliability record.
Postmortem template
Every recorded incident will disclose, at minimum:
- Detection time, source, and the affected surface.
- The signal dimension that failed (data mode, connectivity, freshness, control, or evidence).
- Scope: which records, keys, providers, or claims were affected.
- Corrective action and the independent evidence that the action passed.
- Whether any anchored proof was revoked or superseded.
